About Me and My Office
I have a master’s degree in Social Work and am licensed by the State of Arizona as a Licensed Clinical Social Worker (LCSW). This qualifies me to practice independently. To maintain my license, I must have 30 hours of continuing education every 2 years.
I have a solo practice and operate independently of the other therapists in my office.
My practice is limited to treating adults. I specialize in Cognitive Behavioral Therapy for anxiety disorders: generalized anxiety; health worry (hypochondriasis); panic attacks and agoraphobia; fear of driving, flying and other phobias, (e.g. heights); social anxiety, including fear of public speaking; unwanted disturbing thoughts (OCD); insomnia, and depression.
Office Hours
My office hours are Monday through Thursday from 10:00 am to 6:00 pm. I strive to return phone calls the same day, but sometimes it is the next day. I do not return calls on weekends. NOTE: When returning calls away from the office, I block my number.
Session Fees
The initial 75-minute consultation is $275. Thereafter the fee is $190 per 60-minute session. When longer appointments are clinically indicated, the fee is prorated at the rate of $190 per hour. Fees are payable at the time of the appointment by check or credit card (which I keep on file).
Cancellation Policy & Fees
- Cancellation Without 48-Hr Notice: $150
- No-Show: $190
- Credit Card Decline: $25
- A 3-Hour Notice is required to switch an in-office session to online without incurring the late cancellation fee of $150.
- Completion of employer or insurance paperwork per 30 minutes: $100

Virtual Sessions
In addition to seeing clients in the office, I offer sessions on Zoom to accommodate clients who are temporarily ill or reside/work far from my office. NOTE: Changing your appointment from in-office to online requires a 3-hour notice to avoid the late cancellation fee of $150.
INFORMED CONSENT
Confidentiality, Duty-To Warn, Reports of Abuse
Generally, the information discussed between a therapist and her client is privileged (i.e., confidential), and can only be released with your written permission. The exceptions to confidentiality are:
- If it comes out during a session that you may harm yourself or someone else, I am legally required to break confidentiality to protect you or alert someone you have threatened.
- I am also mandated by law to report any suspected abuse or neglect of a minor or vulnerable older adult. I must report this information to the police, or Arizona Department of Child or Adult Protective Services.
- I may ask you to sign a “Consent” form if I think it would be helpful to share clinical information with a doctor, another therapist, or some other party to coordinate your care.
CLIENT RIGHTS
- To be assured that the information you share is kept confidential, (with the exceptions explained above).
- To receive an explanation of your diagnosis and the treatment plan.
- To collaborate in decisions regarding your treatment.
- To receive an estimate regarding the cost of treatment.
- To address any concerns about your treatment with me.
- To have access to your records pursuant to the HIPAA law.
- To be notified, in the event I terminate my practice, about the location of your records and how you could access them.
CLIENT RESPONSIBILITIES
- To provide notice when cancelling or altering your session, or pay the late fee described in Cancellation Policy & Fees section above.
- To pay for treatment at the time of your session.
- To provide accurate information.
- To actively participate in your treatment, including work between sessions.
- To notify me if you decide to discontinue your treatment.
NOTE: I reserve the right to discontinue treating clients who repeatedly miss appointments or obstruct treatment.
